Patient Injury Rate Number of patient injury events that occurred for every 1000 inpatient days. For example, a rate of 0.5 means that 1 injury occurred for each 2000 inpatient days.
Elopement Rate Number of elopements that occurred for every 1000 inpatient days. For example, a rate of 0.25 means that 1 elopement occurred for each 4000 inpatient days.
Medication Error Rate Number of medication error events that occurred for every 100 episodes of care (duplicated patient count). For example, a rate of 1.6 means that 2 medication error events occurred for each 125 episodes of care.
Prevalence of COPSD Prevalence of all patients served during the time period who are reported with co-occurring psychiatric and substance disorders (COPSD).

30 Day Re-admit Rate (Discharge Cohort)

Percent of discharges from the facility that returned within 30 days of a discharge of the same patient from the same facility. For example, a rate of 10.0 means that 10% of all discharges were readmitted within 30 days.

Patient Seclusion and Restraint

Hours of Restraint Use

Number of hours patients spent in restraint for every 1000 inpatient hours – excludes manual holds less than 5 minutes. For example, a rate of 1.6 means that 2 hours were spent in restraint for each 1250 inpatient hours.

Percent of Patients Restraint Percent of unique patients who were restrained at least once – excludes manual holds less than 5 minutes. For example, a rate of 4.0 means that 4% of the unique patients served were restrained at least once.
Hours of Seclusion Use Number of hours patients spent in seclusion for every 1000 inpatient hours. For example, a rate of 0.8 means that 1 hour was spent in seclusion for each 1250 inpatient hours.
Percent of Patients Secluded Percent of unique patients who were secluded at least once. For example, a rate of 3.0 means that 3% of the unique clients served were secluded at least once

Patient Discharge Survey Results

Patient Outcome Percent of patients at discharge or at annual review who respond positively to the Client Perception of Outcome of Care domain on the Inpatient Consumer Survey.
Patient Dignity Percent of patients at discharge or at annual review who respond positively to the Client Perception of Dignity domain on the Inpatient Consumer Survey.
Patient Rights Percent of patients at discharge or at annual review who respond positively to the Client Perception of Rights domain on the Inpatient Consumer Survey.
Patient Participation Percent of patients at discharge or at annual review who respond positively to the Client Perception of Participation in Treatment domain on the Inpatient Consumer Survey.
Hospital Environment Percent of patients at discharge or at annual review who respond positively to the Client Perception of Facility Environment domain on the Inpatient Consumer Survey.

Hospital Measures

Seclusion, total number of Total number of patient seclusions that occurred, on all units, during the period.
Seclusion, total time (hrs.) Total number of all patient seclusion hours that occurred on all units, during the period.
Restraint, total number of Total number of patient restraint that occurred, on all units, during the period.
Restraint, total time (hrs.) Total number of all patient restraint hours that occurred on all units, during the period.
Court Ordered Medications The number of patients started on Court ordered medication, during the period.
180 Day Adult Re-admit Rate (%) Percent of unique Adult discharged patients from the facility that returned within 180 days (6 months) of their last discharge date from the facility.
Nurse Mandatory OT (hrs.) The sum of calendar quarter’s mandatory nursing hours required for direct patient care including coverage for the Nursing Shift Supervisors Office and admitting responsibilities.
Nurse Voluntary OT (hrs.) The sum of calendar quarter’s voluntary nursing hours required for direct patient care including coverage for the Nursing Shift Supervisors Office and admitting responsibilities.
Nurse Vacancy Rate (%) The Nurse vacancy rate reflects the percentage of fulltime unfilled or open positions compared to the total budgeted positions during this quarter.
Patient Assaults Total number of assaults by patients towards others, facility wide, during the period (including assaults towards other patients, visitors, and staff).
All Staff Injuries Total number of injuries reported by all staff members; facility wide, on facility grounds, or off facility grounds during the course of state business.
Staff Injuries During Patient Assault Episodes Total of all injuries reported and sustained by staff, during patient assault episodes, facility wide, during the period.
