Why Use an Asthma Action Plan?
Asthma is a multi-faceted disease process comprised of multiple physiological components:
- Bronchoconstriction (constriction of the bronchial air passages)
- Airway Inflammation
- Airway Hyperreactivity
- Increased Mucus secretion
Patients whose asthma is not under control may experience exacerbations, altered self-perception, a reduced or compromised quality of life and potentially, remodeling of the airways.
The IAAP is a tool to help your patient and you manage and prevent asthma symptoms while providing information that is crucial to those who come in contact with or care for your patient.
Who Receives a Copy?
The action plan you are about to create is meant to be shared with any individual who may in some way be responsible for caring for or assisting to care for your patient.
Examples of individuals who should have a copy of the IAAP are:
- School Nurses
- Other Specialty Medical Providers
- Daycare Providers
- Parents and family
- Coaches
- Anyone who may be responsible for providing emergency care for your patient
- And - The Asthma patient
Please be aware that any educational information you provide to your patient, including this IAAP is meant to be a supportive tool and not a replacement for individualized teaching and consultation! These guidelines are general guidelines and are not intended to be followed strictly in every individual patient. It is recognized that specific therapies must be customized to each individual patient and that providers may deviate from these guidelines. ]

The Minnesota Department of Health (MDH) Asthma Program, through a grant from the Centers for Disease Control and Prevention, developed this web based, interactive asthma action plan.(www.health.state.mn.us/asthma)
This interactive Asthma Action Plan (iAAP) is a computerized clinical decision support tool for licensed health care providers who treat patients who have asthma. It is not a diagnostic tool and should only be used when the provider has confirmed a patient has asthma. The iAAP follows the NIH, EPR-3 Asthma Guideline 2007 recommendations to assess asthma patient’s severity or control level.
