Short Class Descriptions

Advanced Care Coordination: For Care Coordinators currently certified for 24 months or more only. Topics include but are not limited to: Care Coordination from the individual and family perspective, person centeredness, community resources, review of regulations, documentation, and providing Care Coordination in partnership with SDS.

Agency Based PCA Orientation: The basic requirements to apply for certification as a PCA service provider agency. Topics include service principles, service definitions, the definition of Agency Based PCA, responsibilities as a provider, and interfacing with SDS supports. Required to apply for certification as a provider agency.

Consumer Directed PCA Orientation: The basic requirements to apply for certification as a PCA service provider agency. Topics include service principles, service definitions, the definition of Consumer Directed PCA, responsibilities as a provider, and interfacing with SDS supports. Required to apply for certification as a provider agency.

Basic Care Coordination: The basics of providing the service of the coordination of care. Topics include service principles, understanding the plan of care, and interfacing with SDS supports.

Residential Supported Living in an Assisted Living Home: The basic requirements for providing residential service in an Assisted Living Home. Topics include service principles, service definitions, understanding a resident’s circle of care, responsibilities as a provider, and interfacing with SDS supports.
